Vážený pane Špačku,
video o záchraně a oživování Vašeho syna mi bylo doručeno poštou.
Při prostudování videa i při detailním rozlišení jsem nemohl rozpoznat
žádné
opatření ke stanovení diagnózy pro nouzový stav "zástava srdce a dýchání".
Po prvním nepromyšleném pokusu zdravotnice první pomoci byla poloha zraněného
změněna, aby bylo prováděno oživování třetím zdravotníkem - mužem ca.
1/2 m na
straně závodní dráhy, přičemž toto následovalo bez jakéhokoliv zajištění
hlavy
a nebezpečí zranění krční páteře při úraze tohoto druhu. Je patrné, že
zdravotník nechal hlavu pacienta spadnout, aniž by obě zdravotnice podložily
tělo.
Následně byly zraněnému sejmuty brýle, zdravotnice první pomoci se pokusila opětovně otevřít zapínání helmy a sejmout pak samotnou helmu. Bez známek kontroly dýchání a tepu byla druhou zdravotnicí zahájena "masáž srdce". Byla zdravotnicí rovněž zahájena i komprese hrudníku? Při očividně nasazeném chrániči hrudníku je nutno pochybovat o efektivitě srdeční masáže, není ale především pravděpodobné, že se touto masáží vytvoří dostatečný ventilační objem plic, zejména pak je polohou hlavy velice pravděpodobné uzavření dýchacích cest jazykem. Vaše pochybnosti o efektivitě první pomoci na základě videa nelze vyvrátit, přikládám Vám podklady z online "návodu na první pomoc" rakouského červeného kříže týkající se stanoviska k argumentům potřebného sejmutí helmy, ... které pro jsou pro laiky srozumitelně zobrazeny.
K dalšímu postupu ošetření nemám k dispozici žádné
podklady, na zaslaném CD je
videomateriál dvakrát nahrán.

pro
laiky, postup lékařského personálu a další aspekty resuscitace, podle

Pokud bezprostředně po 2 minutách zaznamenaných na videu bylo prováděno efektivní umělé dýchání, dalo by se i po této nezdařené první pomoci očekávat, že nenastane poškození mozkových buněk z důvodu tepeného nedostatku kyslíku, od 3 minut však již dojde po této stránce k nevratným škodám.

Subject: Herrn Spacek persönlich, Beurteilung des Videos
Sehr geehrter Herr Spacek,
Das Video über Bergung und Reanimation ihres Sohnes wurde mir per Post
zugestellt.
Beim Studium des Videos habe ich auch in der Einzelbildauflösung keine
Massnahmen zur Erstellung der Notfalldiagnose "Herz- und Atemstillstand" erkennen
können. Es wurde nach planlosem Versuch der Ersthelferin. die Position
des Verunglückten zu verändern eine "Bergung" durch 3
Sanitäter von
ca 1/2 m zur Seite der Piste durchgeführt, wobei dies ohne jede Sicherung
des
Kopfes und der bei einem derartigen Unfall höchst gefährdeten HWS erfolgte.
Es
scheint, dass der männliche Sanitäter den Kopf des Patienten fallen gelassen
hat, bevor die beiden Helferinnen den Körper niedergelegt haben.
Anschließend wurde die Brille des Verunglückten entfernt, die Ersthelferin versuchte nochmals den Verschluss des Helms zu öffnen und dann alleine den Helm zu entfernen. Es wurde dann ohne ein Zeichen einer Atmungs- oder Pulskontrolle die" Herzmassage" durch die Zweithelferin begonnen, eine zusätzliche Thoraxkompression durch die Ersthelferin wurde ebenfalls begonnen????
Bei offensichtlich liegendem Brustpanzer ist die Effektivität der Herzmassage anzuzweifeln, vor allem ist es nicht wahrscheinlich, dass durch diese Massage genügend Lungenventilationsvolumen aufgebaut werden kann, zumal durch die Lagerung des Kopfes eine Verlegung der Atemwege durch die Zunge sehr wahrscheinlich ist Ihre Zweifel an der Effektivität der Erstversorgung können auf Grund des Videos nicht zerstreut werden, anbei sende ich ihnen Unterlagen aus dem online" Erste Hilfe Leitfaden" des österreichischen Roten Kreises betreffend der Stellungnahme zur Begründung der notwendigen Helmabnahme,...welche für Laien sehr verständlich dargebracht werden.
Zur weiteren Versorgung stehen mir leider keine Unterlagen zur Verfügung, auf der übermittelten CD wurde das Videomaterial doppelt aufgespielt. Falls unmittelbar nach den im Video festgehaltenen 2 Minuten eine effektive Beatmung durchgeführt wurde istv durch diese missglückte Erstversorgung keine Schädigung der Hirnzellen durch arteriellen Sauerstoffmangel zu erwarten, ab drei Minuten kommt es auch von dieser Seite zu irreversiblen Schäden.

Dear Mr Špacek,
the video recording rescuing and resuscitation of your son was delivered
by the post to
me.
After watching it (even in the highest resolution) I couldn´t detect
any steps to
diagnoze the emergency conditions „cardiac arrest and apneusis“.
After first inconsiderate attempt of the aid woman, the position of injured
Michael was
changed in order to start resuscitation by third aid man – by a man who
was located about
0,5 m far away on the side of the race track. This followed without any
immobilization of
head and prevention from cervical spine injuries that could happen during
such accident. It is
evident that the aid man left the patient´s head fall, without bottoming
the body by the aid
women.
The glasses were taken off the injured person afterwards and the first aid woman tried again to open the helmet and take it off. Without any evident checking of respiration or taking pulse, the second aid woman started „cardiopulmonary resuscitation“. Did this aid woman also start the chest compression?
Because injured was evidentlly wearing the chest-guard, there are reasonable doubts about the effectiveness of the heart massage. But above all, it is unlikely that with this heart massage there would be a sufficient lungs´ ventilation volume set up. And especially head position most likely allows obstruction of the airway with the tongue.
Your doubts about the effectiveness of the first aid can´t be refused on the base of this video. I´m attaching the online „first aid manual“ from Austrian Red Cross relating to the attitude towards the arguments of the necessity of the helmet removal ... that are simply pictured for the nonspecialists:
I don´t keep any records about following attending steps in my disposal. There is the videomaterial recorded twice on the sent DVD. There is an article about resuscitation on the platform Wikipedia and it´s steps are described for the nonspecialists. It also contains description of the steps of the medical personnel and other aspects of the resuscitation that may allow you review the following medical attendance.
